 По оценкам ВОЗ в настоящее время около 400 миллионов людей во всем мире страдают от диабета, поэтому очевидно, что исследователи постоянно стараются разработать методы лечения этой болезни.
Исследователям из Университета Северной Каролины удалось разработать новый подход к терапии диабета – автоматизация введения инсулина в организм диабетиков. Исследователи разработали «пластырь», который высвобождает инсулин в кровь, реагируя на изменения концентрации кислорода, связанные с увеличением содержания глюкозы. Есть надежда, что новая разработка станет основой для более интеллектуального и безболезненного способа терапии диабета.
Основой разработанной системы являются самоорганизующиеся везикулы с инкапсулированными глюкозооксидазой и дозой инсулина. Самоорганизация везикул обеспечивается строением блоков, из которых они построены – внешний слой наноструктур образует гидрофильная гиалуроновая кислота, защищающая гидрофобные нитроимидазольные боковые группы.

В нормальных физиологических условиях везикулы устойчивы, однако они легко разрушаются в присутствии избыточного количества глюкозы. Механизм раскрытия везикул таков – оксидаза способствует окислению глюкозы до глюконовой кислоты, при этом расходуется кислород. Понижение концентрации кислорода внутри везикулы приводит к тому, что внутренние гидрофобные группы везикулы приобретают гидрофильность, что приводит к диссоциации наноконтейнера и высвобождению инсулина.
Исследователи создали терапевтический пластырь, введя везикулы в биологически совместимые микроиголки, которые безболезненно прокалывают кожу при его применении. Пластырь был опробован на мышах, больных диабетом, испытания показали, что один такой пластырь существенно улучшает их состояние на девять часов.
Как отмечает один из авторов работы, Жен Гу (Zhen Gu), это исследование не является первой разработкой в области доставки инсулина, регулируемого содержанием глюкозы, тем не менее, это первый пример использования для решения данной задачи материалов, чувствительных к содержанию кислорода. Хотя ферментативная конверсия глюкозы в глюкуроновую кислоту и использовалась ранее, прежние системы главным образом реагировали на изменение уровня pH, однако pH-чувствительные везикулы отличаются гораздо более медленной реакцией на изменение окружающих условий.
В стратегических планах исследователей разработка пластыря, который был бы способен доставлять инсулин не менее двух-трех дней. На настоящий момент уязвимым местом нового медицинского «пластыря» является количество инсулина, которое может быть загружено в микроиголки, но исследователи предполагают, что это решаемая проблема. Также они планируют испытать пластырь на микро-свиньях, структура кожи которых ближе к коже человека, чем кожа мышей.
